What this covers, and what it can't
Part B premiums and IRMAA surcharges are set nationally by CMS and Social Security, so they can be calculated precisely from your income and filing status. That is what this estimates, using 2026 figures assessed on your 2024 return.
What it can't estimate is plan pricing. Medigap premiums, Medicare Advantage costs, and Part D plan premiums vary by county, by carrier, and by your specific medication list. Any figure a website invented for those would be wrong for most readers, so this tool shows them as varying rather than guessing.
If the surcharge looks wrong
It may be. IRMAA is assessed on a tax return roughly two years old, so someone who retired recently is often charged on income they no longer earn. A qualifying life-changing event lets you ask Social Security to recalculate — how the appeal works.

Common questions
How much will Medicare cost me each month?
Most people pay the standard Part B premium, plus an income-related surcharge if their income exceeds the threshold. On top of that you would have any Medigap or Medicare Advantage plan premium and any Part D plan premium, which vary by county and plan.
Does IRMAA apply to me?
IRMAA applies if your modified adjusted gross income from the relevant tax year exceeds the threshold for your filing status. The calculator above shows which tier your income falls into.
Why does Medicare use income from two years ago?
Social Security determines IRMAA using the most recent tax return available when premiums are set, which is generally two years prior. That is why recent retirees are often assessed on their working-year income.
Can I lower my Medicare premium?
If a life-changing event reduced your income — retirement, the death of a spouse, divorce, or loss of a pension — you can ask Social Security to recalculate using Form SSA-44. Medicare Savings Programs and Extra Help may also assist those with limited income and resources.
